New Patients:

Welcome New Patients.

Thank you for choosing Centrepoint Dental Centre as your premiere Dental service provider.  We would like to take this opportunity to provide you with some information that you will find most helpful before your visit.

Please ensure that you are healthy before making or keeping your appointment with us.  If you are not, please advise us as soon as possible so we can determine if it is safe to keep your appointment with us.

Your First Visit:

We highly recommend that you provide our office with your Dental Insurance information when you make your appointment with us.  Allowing us to have your insurance information ready and on hand when you come into our office.  We further recommend that on the day of your appointment, that you come to our office 15 minutes ahead of your appointment time.  This additional time will be taken by filling out your chart with your personal information such as address, insurance information and personal health history. 

Missed or Cancelled Appointments:

Please note that there is a $50 charge for missed appointments.  This charge also applies to appointment cancellations where less than 24 hours notice is given.  However, we do understand that sometimes life can throw you a surprise, therefore it is always recommended that if you cannot make an appointment, to call us as soon as possible, since the charge may be forgiven.  In the event that our office is closed, please leave a message, since it does count towards your notification to our office about the cancellation.


Insurance Information:

If your insurance provider is one of the following companies, please be sure to bring your benefits book with you when you come for your first visit:

  • GreenShield
  • Liberty Health
  • Standard Life

Please note:  Due to privacy law changes that took effect January 2004, our office has limited ability to obtain Insurance plan details.  All financial maximums, including the status of your account with your insurance provider and other private information is protected by your Insurance company, and said information will not be disclosed to us without your expressed permission before hand.

On your behalf, we are pleased to submit forms for Insurance claims.  We accept direct settlement from Insurance plans, which means you, will be asked to pay only your portion of the total fee.  At all times, however, you are responsible for all fees and costs not settled by your Insurance plan.
